This notice is provided for transparency regarding health information handled in connection with services offered through Live Now MD. Your treating clinicians and covered entities may have separate notices where applicable.

We may use and disclose PHI for purposes including:

Our duties

We are required by law to maintain the privacy of PHI, provide this notice, and follow the terms of the notice currently in effect. We reserve the right to change this notice and to make the revised notice effective for all PHI we maintain.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us at hello@livenowmd.com or with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
